Steps to Get Your ID
- You will need the email address you registered with and your birthdate.
- Click on the link above, it will take you to the registration ID request page.
- Enter your date of birth and the email you registered with.
- Click Save and Submit. You will return to this screen.
- IF the system finds one or more records that match the registration IDs associated with those email addresses will be sent to that email address. If at 24 hours you don't receive an email, then no exact match has been found in the system. Be sure to check your junk or spam email folder. The email comes from quickbase.com.
If you did not register using an email address, cannot remember which email address you used or you try the registration ID request and you don't get an email, then try either
- Calling the COVID-19 call center at 817-248-6299
- Sending an email to covidshot@tarrantcounty.com, and requesting your ID# be emailed to you. You must indicate that you have tried to use the self look up tool.

What is a Registration ID
Your registration ID number is the number you will use to check your status and check in at the site.
Check Your Status
Steps to Complete a Status Check
- Click on the blue button above, it will take you to the status check page.
- Enter your registration ID number and birthdate.
- If you only want to know your status, and don't want to reschedule or cancel your appointment indicate "No Change" for the question "Do you want to cancel or reschedule?"
- Click Save and Submit. You will return to this screen.
- If the Registration ID number and the birthdate match what is in our records you will be emailed an update on your registration and appointment status. It will be emailed to the address you used at registration.If after 24 hours you don't receive an email, then no exact match has been found in the system. Be sure to check your junk or spam email folder. The email comes from quickbase.com.

What information is available
The following information is available from the status check:
- The date that you registered.
- Vaccine eligibility based on Texas 1a and 1b requirements.
- Estimated wait time to your appointment.
- Appointment status/details.
- Phone number on file to contact you.
Reschedule or Cancel
Steps to Modify Your Appointment
- You only need your registration ID number and your birthdate to check cancel your appointment.
- Click on the blue button above, it will take you to the status check page.
- Enter your registration ID number and birthdate.
- Answer the question "Do you want to cancel or reschedule?" Indicate if you want to cancel your registration or just modify your appointment.
- Click Save and Submit. You will be forwarded to a confirmation page indicating that your request has been submitted.
- If the Registration ID number and the birthdate match what is in our records your request will be processed and a confirmation will be emailed to you. It will be emailed to the address you used at registration. If after 24 hours you don't receive an email, then no exact match has been found in the system, and we are unable to process your request automatically. Be sure to check your junk or spam email folder. The email comes from quickbase.com.

Which one do I want to do
If you obtained vaccination elsewhere, then cancellation is the choice you are looking for. If you cancel, you are cancelling your registration and you will not be given another appointment.
Most Tarrant County Public Health vaccination sites will still honor your appointment even if you show up after the date it has been scheduled as long as you go to the site you were originally scheduled at AND they have the capacity to see you. If that site is no longer operating then you will need to reschedule.
Please note that on certain days sites may be only offering first or second doses, and may request that you come back another day.
