Tarrant County Public Health COVID-19 Shot Sign Up

Step 1: Sign Up

Please make sure the information you enter is correct. You will not be able to change the information after you hit submit. Once submitted, you will see a confirmation page.
Each person who wants to get vaccinated must complete the sign up process using their own email address. If you only have one email address for the family, please create additional emails for each person. Learn more about free email accounts at: https://blog.hubspot.com/marketing/free-email-accounts.
Complete the sign-up form by clicking on the blue "Click Here to Sign Up for a Covid Shot" button.
Do Not sign up more than one time The system will delete duplicate entries. If you received an email confirming you signed up to get a vaccination or your first dose was provided by TCPH, we will contact you for all appointments. Please call the COVID-19 Hotline at 817-248-6299 or email covidshot@tarrantcounty.com for assistance with changes to your contact information or if you missed your appointment.
