Welcome to the Tarrant County Public Health COVID-19 Test Kit Requesting Form

Tarrant County Public Health is now offering Free At-Home COVID-19 Test kits. Anyone can request for self-pick up of up to 5 test kits per residential address. All locations are open for pick up Monday – Friday 8:30 a.m. – 11:30 a.m. & 1:30 p.m. – 4:30 p.m. Please fill out the information below to place your request. If you are having trouble filling out the below form, please call 817-248-6299 for assistance.

Note: If 5 test kits were already distributed to your household, you can still go online and request from the USPS at COVID Home Test | USPS
